Things to know before filling out this form:
- Proof of Rabies vaccination is required – email to [email protected]
- Proof the dog is spayed/neutered (if applicable)
- License fee of $6.00 for dogs that are spayed/neutered or $15.00 for dogs that are not
- Please call the Town Clerk’s Office for anymore information or questions at (585)624-6060.
Your license and tags will be mailed to you within 1-2 business days from the date you submit the form.
